You get the 2 year anniversary award, since it has not yet been 5 years. 
And referrals before the application was applied unfortunately do not count.
The rest you qualify for and I'll award them now (Oh, you only get one anniversary award because post count, anniversary awards, etc simply replace the previous milestone)

They have an account but they didn't use the link - does it only count if they click the link?

they have to use your name in the referral box, or else there is no way to know if you actually referred them.

They have to enter your name as a referrer when they sign up so that it will show up in the mod for approval.
